Community support
Community and outreach support in West Essex.
Some people do not live in a supported living scheme, but still need reliable help at home and out in the community. Outreach visits are planned around the person’s week, not around a building’s rota.
What this support looks like
A support worker visits the person at home, meets them in the community, or both. The aim is the same as our supported living work: help the person do more of what matters, safely, and with dignity.
What it can include
At home
Help with daily living skills, routines, cooking, shopping, looking after a tenancy and building confidence in ordinary tasks.
Out in the community
Support to get to college, appointments, work, volunteering, leisure, places of faith and the people who matter.
Hours that can grow or change
Support is typically a few hours a week upwards. Evenings and weekends can be part of the plan where that is assessed and commissioned.
Personal care, and support that does not include it
Some community support is practical help with daily life, travel, appointments or community access. That kind of support may not include personal care.
Personal care means help with washing, dressing, oral care, continence and similar daily routines. It is only part of the plan where it is assessed as needed. The same standards of listening, consistency and dignity apply either way.
Still a small, consistent team
Outreach is not a stream of different people each week. We organise support in local clusters across Uttlesford, Epping Forest and Harlow so the person is known.
How people usually access support in Essex
Essex County Council Adult Social Care can assess what support a person may be eligible for. Community support may be commissioned on its own or alongside other support.
